Private home healthcare, specializing in medical and non-medical care for older adults and disabled adults needing additional assistance wherever their home is for them * in-facility relief staffing services
* Develops and implements customized care plans
Services include, but not limited to:
- personal care supported
- medication assistance and monitoring
- respite care
- hospital to home transition
- wellness checks with Personal Support Worker for medication checks and safety
- palliative care
- advocacy and coordination of medical and social services
- homemaking services including meal preparation, laundry, errands and light housekeeping services
- assistance with activities of daily living including accompaniment and transportation to local appointments for clients
- companionship
- range of Motion exercises
- Alzheimer's and related dementia care by "Certified Dementia Practitioner"
- Motor Vehicle Accident Care Programs
Approved Registered Veterans Affairs, Canadian Forces, WSIB and insurance claims,
Primer Accredited by Accreditation Canada.
