Description des services:
|
Community-based service for youth and young adults with a developmental disability, which will assist them in the planning and transition process from school to life as adults in the community
Services include:- access to a transition coordinator
- supports and resources based on needs and interests
- assist in completing application process for Ontario Disability Support Program
- assist in completing referral package and application for Developmental Services Ontario
- in conjunction with the school system, completion of Integrated Transition Plan and updated on a yearly basis
- connect to resources for skill building, recreation and leisure
- assist with application to Disability Tax Credit and Canada Disability Benefit
